BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2014208495 DESCRIPTION An 86-year-old man presented to accident and emergency with symptoms and signs of a lower respiratory tract infection. He had a history of severe dysphagia and regurgitation of food associated with two-stone weight loss and a significant reduction in quality of life. The patient was well apart from recent coronary artery bypass graft surgery. He was treated successfully for aspiration pneumonia and a subsequent chest X-ray revealed a mediastinal fluid level consistent with a possible pharyngeal pouch (figure 1). The patient was referred to the Ear, Nose and Throat clinic. Full examination of the head, neck and oral cavity (including flexible nasoendoscopy) revealed no abnormalities. A barium swallow confirmed a large pharyngeal pouch (figure 2). The patient went on to have successful endoscopic stapling of the pharyngeal pouch with an improvement in symptoms. Pharyngeal pouches typically occur in patients aged over 70 years and can present with dysphagia, regurgitation, chest infections, aspiration, halitosis and loss of weight. 2 A barium swallow usually diagnoses a pharyngeal pouch. Pharyngeal pouches are managed with endoscopic stapling or open surgery. While a barium swallow test is the gold standard radiological investigation for a pharyngeal pouch, this unusual case illustrates the versatility of a plain chest radiograph in demonstrating a variety of pathology.

An unusual complication of percutaneous endoscopic gastrostomy (PEG) placement in the presence of a large pharyngeal pouch.

endoscopic gastrostomy (PEG) placement in the presence of a large pharyngeal pouch An 80-year-old man with a known benign lower esophageal stricture and pharyngeal pouch was admitted with a 2-week history of malaise, dysphagia, regurgitation, and weight loss. He had elected not to have the esophageal stricture dilated 3 years earlier. On this admission, the dysphagia persisted despite dilatatio...

Relationship between posterior pharyngeal pouch and hiatus hernia.

The formation of posterior pharyngeal pouches is generally attributed to dysfunction of the cricopharyngeal sphincter. The reason for this dysfunction and its exact nature have not been established. Observations in the Royal Victoria Hospital, Belfast, suggested that an association exists between pharyngeal pouch and hiatus hernia. Barium studies performed in patients with a pharyngeal pouch us...

Reiterative expression of pax1 directs pharyngeal pouch segmentation in medaka.

A striking characteristic of vertebrate development is the pharyngeal arches, which are a series of bulges on the lateral surface of the head of vertebrate embryos. Although each pharyngeal arch is segmented by the reiterative formation of endodermal outpocketings called pharyngeal pouches, the molecular network underlying the reiterative pattern remains unclear. Here, we show that pax1 plays c...

An essential role for Fgfs in endodermal pouch formation influences later craniofacial skeletal patterning.

Fibroblast growth factor (Fgf) proteins are important regulators of pharyngeal arch development. Analyses of Fgf8 function in chick and mouse and Fgf3 function in zebrafish have demonstrated a role for Fgfs in the differentiation and survival of postmigratory neural crest cells (NCC) that give rise to the pharyngeal skeleton. Here we describe, in zebrafish, an earlier, essential function for Fg...

The Morphology and Pathology of the Pharyngeal Pouch of Rathke

The pharyngeal pouch or bursa pharyngea was first described by Rathke in 1838.1 It is now a place of great interest both to the surgeon and to the scientist: here post-nasal adenoids have their birthplace, and probably the oesophagus has its ancestry. The pouch of Rathke is a tubular recess at the upper and posterior wall of the pharynx, not at all infrequent in the adult, commoner in children,...
